Support Health Improve ("Green Leaf," "we," "us") provides home health and wellness management services — including online doctor consultations, family medicine, preventive care, annual checkups, health screenings and mental health counseling — throughout the United States. 2. Information We Collect

2.1 Health Information (Protected Health Information, "PHI")

Medical history, symptoms, medications, allergies and treatment plans you share with our providers.
Appointment records, consultation notes, lab and screening results, prescriptions and referrals.
Insurance, superbill and membership billing details related to your care.

2.2 Personal & Contact Information

Name, email address, phone number, mailing address and date of birth.
Emergency contact details you voluntarily provide.

2.3 Technical Information

IP address, browser type, device information and pages visited, collected via first-party analytics to improve site performance.
Cookies and similar technologies (see Section 7).

2.4 Newsletter Data

If you subscribe to our newsletter, we store your email address solely for that purpose. Every email contains a one-click unsubscribe link, and you can also unsubscribe at unsubscribe.html.

3. How We Use Your Information

Treatment: to provide, coordinate and document your medical and mental-health care.
Payment: to process memberships, generate superbills and administer refunds.
Operations: to improve our services, train staff (de-identified data only) and maintain security.
Communication: appointment reminders, care instructions and — only with your consent — wellness content.
Legal: to comply with applicable federal and state law.

We do not sell, rent or trade your personal or health information. We do not use your PHI for advertising.

4. HIPAA Notice of Privacy Practices

As a covered entity under the Health Insurance Portability and Accountability Act (HIPAA), we may use and disclose your PHI for treatment, payment and healthcare operations without additional authorization. Other uses — such as most marketing or any sale of PHI — require your written authorization, which you may revoke at any time. You have the right to:

Request access to and copies of your medical records (fulfilled within 30 days).
Request amendments to incorrect records.
Receive an accounting of disclosures.
Request confidential communications (e.g., alternate phone or email).
Request restrictions on certain uses and disclosures.

5. How We Share Information

We share information only in these limited circumstances:

Your care team: providers, nurses and counselors involved in your treatment.
Service providers (Business Associates): secure hosting, laboratory and pharmacy partners bound by HIPAA Business Associate Agreements.
Legal requirements: court orders, public-health reporting or threats to health and safety, as required by law.
With your consent: any other sharing, only with your explicit, revocable permission.

6. Data Security

We protect your data with AES-256 encryption at rest, TLS 1.3 in transit, role-based access controls, multi-factor authentication for staff, routine penetration testing and 24/7 intrusion monitoring. Despite our safeguards, no system is perfectly secure; we commit to notifying affected individuals and regulators within legally required timeframes in the event of a breach.

7. Cookies & Tracking

We use strictly necessary cookies (session security), preference cookies (e.g., billing-mode selection) and anonymized analytics cookies. We do not use cross-site advertising trackers. You may disable cookies in your browser; core features will still function.

8. Data Retention

Medical records are retained for the period required by Idaho and federal law (generally at least 7 years after the last episode of care). Newsletter data is deleted within 30 days of unsubscription. Website analytics are anonymized after 14 months.

9. Your Rights (Including CCPA for California Residents)

Know, access, correct, delete: request details of, copies of, corrections to, or deletion of your personal information.
Opt out of "sale/share": we do not sell or share personal information for cross-context advertising — ever.
Non-discrimination: exercising these rights will never affect the quality or price of your care.

10. Children's Privacy

We do not knowingly collect information from children under 13 except through a parent, guardian or Family Circle membership holder acting on their behalf. Parents may review or delete a minor's information by contacting us.

11. Third-Party Links

Our website may link to external resources (e.g., health education sites). We are not responsible for their privacy practices and encourage you to review their policies.

12. Changes to This Policy

We may update this policy to reflect legal or service changes. Material changes will be announced by email and on this page at least 30 days before taking effect. The "Effective date" above always reflects the current version.

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health & Human Services Office for Civil Rights. We will never retaliate against you for filing a complaint.
